Unable to upload file

Trying to upload from frontend I've got: "Unable to upload file"
BUT image is uploading on server but not publishing in frontend, in current category.
What is a problem? I've made 777 to images/phocagallery/ and images/phocagallery/thumb

Hi, check permissions but ownership too of your files and folders on your server, check the GD library too:
